چکیده
Background: The symptom burden of typical rheumatoid arthritis (RA) symptoms in those at risk for developing RA - positive anti-citrullinated peptide antibodies (ACPA) and musculoskeletal complaints has not been explored. Objectives: To evaluate patient-reported individuals at-risk to develop comparison people with newly diagnosed, early seropositive RA. Methods: Two datasources from the region Stockholm were used: RISK cohort follows up ACPA arthralgia, but without signs joint inflammation a structured program over 3 years. baseline visit this was used. Patients (ACPA and/or factor positive) (symptom duration 12 months maximum) reported Swedish Rheumatology Quality Register (SRQ) their closest diagnosis date before receiving DMARD treatment have identified. At-risk matched 1:3 by sex age using nearest neighbour method utilizing Mahalanobis distance, corrected sample bias exact matches on sex. Effect estimates being compared pain, patient global (GH), fatigue (all visual analogue scales), health assessment questionnaire (HAQ), TJC28 EuroQol-5D (EQ5D; range: 0-1) derived. Propensity score matching used as sensitivity analyses. Results: A total 223 820 patients. summary distribution variables effect differences between are shown Figure 1. show 24mm lower pain scores than This difference is even less (-17mm, 95%CI: -24 -11) when estimate additionally number tender joints. average 5.4 joints People 22mm GH (rate themselves better) 17mm Fatigue scored highest (mean:35mm, 30 29) among three VAS. HAQ 0.6 points EQ5D showed 0.24 higher index-values, outlining better status. Still mean only 0.74 (95%CI: 0.71 0.77), which values an age-matched general population (0.92 0.96). Sensitivity analyses revealed similar results. Boxplots health, fatigue, TJC28, HAQ, separately displayed at-risk. Population p<0.001) comparing provided every pair, 95% confidence interval brackets. Conclusion: Not surprisingly, report then diagnosed However, these range around minimal clinically important below, stresses need medical attention management strategies symptomatic individuals. متن کاملRheumatoid Arthritis: Early diagnosis and treatment outcomes
Rheumatoid arthritis (RA) is an inflammatory progressive disease which in the absence of appropriate treatment can lead to joint destruction and disability. Prognosis of RA may be predicted based on the presence of some clinical and laboratory evidences.New criteria for classification of RA provides opportunity for earlier treatment. Initiation of treatment particularly by combination of DMARDs...

High levels of rheumatoid factors (RF) are detectable in serum of the majority of patients with rheumatoid arthritis (RA), but 5-10% of patients remain seronegative (SN). Despite clinical and genetic similarities between these two subsets of RA, it has been proposed that they may be regarded as distinct clinical entities. Methods: In the present study a panel of monoclonal antibodies (mAb) rec...
متن کاملThe subgingival microbiomes in periodontitis and health of individuals with rheumatoid arthritis and at risk of developing rheumatoid arthritis
Serum anti-citrullinated protein antibodies (ACPAs), present in 70% of people with rheumatoid arthritis (RA), can be detected ≤10years before the onset of clinical disease. RA and periodontitis are epidemiologically associated and we have reported a high incidence of periodontitis in people who are ACPA+ and at risk of RA. Periodontal bacteria may contribute by multiple routes to the generation...
